Thanks all, I went through the process again and now it's saving the console and other files.
I think the problem may have been in my patience. When installing, I would restart it if there was a really long period of no feedback. I let it work longer this time during all the steps and saw it continue doing more stuff. It might be that I just couldn't expect it to work as fast as I thought.

I found that on my system, it's
cat /sys/class/leds/omega2p\:amber\:system/trigger
And I get this:
root@Omega-B1A3:/sys/class/leds# cat omega2p\:amber\:system/trigger none [timer] default-on netdev transient mmc0 gpio heartbeat morse oneshot
So, I used this:
echo default-on > /sys/class/leds/omega2p\:amber\:system/trigger
After that, the amber light stays lit... until I turn it off and back on. It blinks continually again and I see that my change didn't stick.
root@Omega-B1A3:~# cat /sys/class/leds/omega2p\:amber\:system/trigger none [timer] default-on netdev transient mmc0 gpio heartbeat morse oneshot
This seems like a problem with the flash memory...

I should make it clear that what seems like the bigger issue is that nothing stays installed. I added php and added the basic index.php so I could view it. It worked but when I turned it off and back on, I couldn't get it again. It was gone.

I am having the same issue. The orange led never stops flashing. What did you do to resolve it? What firmware, etc.?

@cusackph You rock. I installed again and it was listed. I did the steps and voila! I'm back in action. Well, I'm back to where I was when the console wouldn't show any apps after restarting. But at least now I can troubleshoot something besides a paperweight.

@cusackph This probably makes me sound like a total n00b but how did you connect the Omega to your USB adapter?
I am guessing you plugged the USB into your computer. Then wires with pins on each end (don't know the official name) to connect the middle two USB adapter wires to the TX and RX holes in your arduino? I don't have the arduino adapter so I'm trying to figure out how to do this with the expansion board. The expansion board has TX+/- and RX+/-. -

The output is the same when the dock is unplugged vs plugged in and lights flashing, during the 30 sec it flashes after turning it on with the reset button pressed. I copied the list and diff'd it to make sure. Here's what I get though:

@cusackph When I use ls /dev/tty* and I get a lot more items (expected).
I'm using the expansion dock, powered from the USB cable attached to my computer.
I restarted but still no luck.Well done documenting your steps in the un-bricking adventure. I've ordered the USB adapter and will push forward when it arrives.

I installled the driver linked to from https://docs.onion.io/omega2-docs/connecting-to-the-omega-terminal.html#connecting-via-serial-mac, but the device never showed up when using ls tty.*. Either the driver didn't install correctly or my Omega2+ is really dead.

Ok. I will search the web for how to do that with my Mac. I'm connected from my Mac USB to the dock mini USB port. If you know the url of a resource on the web, I'd love to hear it.
Currently, when I turn it on while holding down reset, the amber light blinks rapidly for 30 seconds, then goes dark. I'm guessing this is the period where it would be asking to reset.

This sounds like what I need to do. I don't understand where you'll see "option #2" though? Maybe I need to research how to connect via USB to Serial but if you power it on with the reset button down, there must me some steps that will give you options on your screen. Am I missing something?

@Brad-Buskey Hm, so I tried using b150 and it seemed to work. I was able to go to the console page and start through the setup wizard again. It had issues with the wifi step. It showed my wifi networks and I chose the same one I've been using. It said configuring wifi for a really long time so I decided to restart it. I SSH'd to it and ran the install again. This time the upgrade failed writing to the firmware. The lights turned off and wifi doesn't work. When I turn it on, the lights blink for a few seconds, then go dark. I think I bricked it.
I'll look for how to manually fix it if it's possible. For now, I'm going to go drown my sorrows.

@Brad-Buskey After using your downgrade steps, I got this message:
root@Omega-B1A3:~# sysupgrade omega2p-v0.1.8-b148 Image metadata not found Invalid image type. Image check 'platform_check_image' failed.
Hopefully there will be an upgrade that fixes this...

I had a glimmer of hope after using /etc/init.d/console restart. It brought the console back to normal but after a restart, it was back to just showing Index of /console/.
It seems like the memory has issues keeping the files.

Now it's working... so far.The LED still blinks forever though.
UPDATE: After a restart or two, now I get the same thing as Anders. The console comes up and I log in but no apps are displayed. And after another restart, it just get an index of /console/ showing ../ and lib/. If I use ssh to reinstall console it shows again, temporarily.

I have the same issue as Anders. The led never blinked in the beginning but I was able to go through the wizard and when it completed, the led turned off. I restarted as suggested and now the led never stops blinking. I can't access it unless I'm on the omega wifi but in that case, I just get the wizard again. I tried going through the wizard and everything was saved and looked ok. The last page of the wizard said it was successfully set up and please wait while the console is installed. Nothing changes after waiting. The led still blinks forever. Any ideas?
